Investigation Summary
At 5:30 p.m. on February 13, 2018, an employee was tightening and replacing a belt on a drive motor of the air handling unit and had not locked out or tagged the equipment. The employee reached into the belt and pulley area to check the tension and had his finger pulled into a pinch point, where he amputated the tip of his right hand's middle finger. Hospitalization was not required.
