Investigation Summary
At 12:00 p.m. on February 8, 2018, an employee was operating the Haven 3 tube cutting machine. The employee reached inside the machine to adjust the cooling lines located near the machine's point of operation without locking out the machine. The Haven 3 tube cutter cycled while the employee's hand was inside and the blade bracket cut into his hand, partially amputating the employee's right pinky and crushing his ring finger. The employee was hospitalized.
