Investigation Summary
At 9:20 a.m. on February 14, 2018, Employee #1, employed by a heavy equipment rental company, was working at the company's yard. Employee #1, a delivery driver, was working at the loading dock and attempting to drive and load a Genie S-64 4 by 4 Boom Lift onto a flatbed trailer attached to a truck. During this process, the boom arm rose suddenly, and Employee #1 was ejected from the lift basket. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a fractured skull and deep lacerations to his head.
