Investigation Summary
At 9:00 a.m. on January 13, 2018, an employee was transporting cardboard on a Toyota forklift (Model Number 8FBCu20, Serial Number 72102f) for recycling. The employee held a strap with his right hand to keep the cardboard from falling off the forks and the strap became tangled in the forklift's rear wheels, pulling it tight around his middle fingers. The employee amputated the tips of his three fingers and was hospitalized.
