Investigation Summary
At 1:45 a.m. on February 28, 2018, Employee #1, a hammer press operator, removed the punch from the Toledo mechanically powered hot trimming press, without locking out the press. The employee began by loosening one of the two bolts on the face of the press ram that holds the punch in the ram. Employee #1 was using his left hand to work the wrench to loosening the bolts. He used his right hand to hold the punch, so it would not fall from the ram. While loosening the bolt, the employee's left elbow struck the single press control button and cycled the press. As the press cycled, two of his right fingers were caught in the point of operation, between the punch and blade, causing amputation of both fingers.
