Investigation Summary
At 8:15 a.m. on February 12, 2018, an employee was cleaning the water roller of an Akiyama 628 printing press and pressed the control button, putting the rollers into a continuous motion. The employee had his left hand on the roller which was pulled into the roller along with his lower arm. A coworker stopped the machine and Emergency Services was contacted. Emergency Services attempted to free the employees arm and was unable to do so. A surgical unit was brought in from Harbor UCLA and the employees lower arm was amputated onsite. The employee was transported to UCLA Harbor where he was hospitalized.
