Employee's finger is amputated when caught between chain and

At 8:15 p.m. on January 27, 2018, Employee #1 was relieving the machine operator for break. A carton became jammed in machine N7, so the employee reached in the running machine to remove the jammed carton. The employee's right index finger became caught between a running chain and sprocket in the machine, resulting in the amputation of his right index finger at the first knuckle.
