Employee catches finger in nip point and partially amputates

At 2:15 p.m. on February 5, 2018, an employee was changing out a die on a hydraulic cut off machine. The pneumatic air source was not locked out and it activated while the employee had his left ring finger in a nip point. The employee caught his finger in a nip point between the die and steel arm and amputated it. The employee was hospitalized.
