Investigation Summary
At 1:43 p.m. on January 19, 2018, an employee was unloading a new ATM from a pallet and had removed the bolts that were holding it in place. The employee was under the ATM machine that had been elevated by a forklift when it fell off of the forklift's prongs, striking and fracturing the employee's right leg. The employee was hospitalized.
