Investigation Summary
At 6:15 a.m. on January 29, 2018, an employee was working alone, driving to a customer jobsite to deliver a Nar Jib Boom powered industrial platform (model number: LWI2745) for work on an HVAC system. As he unloaded the powered industrial platform, weighing 6,040 pounds, the chain that was rigged to hold the lift slipped. The unit tipped over and fell on him. The employee sustained a broken back, shattered pelvis, and injuries to his knee and shoulder. He was hospitalized for treatment.
