Investigation Summary
At 2 p.m. on February 01, 2018, an employee was testing and adjusting a valve assembly which was mounted on a bench top vise. As the employee approached the valve assembly, his foot became caught in an air hose and electrical wires in front of the bench causing them to lurch forward. The employee extended his right hand in an attempt to catch himself; however, the employee's finger landed in the stainless steel ball pinch point of the valve assembly. The employee's left hand landed on the power supply leads causing the valve assembly to operate and change positions, resulting in an amputation of the employee's right finger tip.
