Investigation Summary
At 12:30 p.m. on December 19, 2017, an employee was setting up a design on the M & R Terminator Textile Screen Printing System. While the machine was on, the employee climbed on one of the safety bars without disconnecting the security cables to adjust the screen. The employee's shirt became caught inside the machine arm, resulting in her shirt catching fire. The employee's back was burned, requiring hospitalization. She was transported to a nearby emergency where she received treatment.
