Investigation Summary
At 2:00 p.m. on December 8, 2017, Employee #1 was in the production area, removing pie lattice dough from tins off of the Colborne 1, Model AD33600B, and Serial Number 1185-95, pie making line as it was rolling the pie tins. The employees left arm became caught in the unguarded crimp head. Her arm was pulled into the machine and crushed. Employee #1 was transported to the hospital where she had surgery.
