Investigation Summary
At 4:45 p.m. on January 24, 2018, an employee was operating a Bliss C-110 power press that was equipped with a pullback operator restraint system, as well as a two push button system. The two button system is over ridden when the press is switched into automatic or continuous mode. Also, the machine was designed to be used as a full cycle machine or be used in jog or inching mode. After using the press to form approximately 400 pieces, he noticed that the press was not forming the pieces properly, requiring adjustment. The employee then called a coworker over to the press that was responsible for performing adjustments, who switched the press to automatic continuous mode without the employee knowing. As the employee reached into the point of operation to retrieve a piece/part after the press completed one full cycle, the die suddenly closed and amputated three of the employees fingers.
