Investigation Summary
At approximately 12:00 p.m. on January 10, 2018, Employee #1 was located on the cab guard of a Terex Hi-Ranger, elevated 8.5 feet, repairing the boom rest saddle of the unit. As he bent over to collect his tools and metal remnants from the repair, Employee #1 stepped on a flat piece of metal. Employee slipped and fell forward off of the front of the cab. He landed on the shop floor below, sustaining multiple injuries. Employee #1 was hospitalized for his injuries. The incident investigation identified that Employee #1 was not wearing fall protection at the time of the incident.
