At 7:30 a.m. on January 22, 2018, an employee was cleaning a hopper and began emptying it. The employee stretched his hand down through a portal and came in contact with operating rotary valve which was 33 inches away. The employee amputated the tip of his right middle and ring fingers. The employee was hospitalized.
