Investigation Summary
At 3:40 p.m. on January 15, 2018, Employee #1 and Coworkers #1, #2, and #3, employed by a construction company, were working at a multiemployer construction project, a new two-story multifamily residential structure. Weather conditions included light snow throughout the day and and temperatures of 20 degrees Fahrenheit and below. Snow covered the building's roof and created a slippery surface. Employee #1 was removing snow from the rooftop while his coworkers were installing roof sheathing. Employee #1 slipped and fell, a fall height of approximately 19 feet. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for multiple fractures to both ankles and feet that required surgery and subsequent physical therapy and rehabilitation.
