Investigation Summary
At 10:00 a.m. on January 15, 2018, an employee was training a coworker on the operation of the fertilizer distribution conveyor belt system. The employee was showing his coworker how to clear the belt of built-up fertilizer while the conveyor belt was running and caught his left arm in the belt, fracturing it in multiple places. The employee was hospitalized.
