Investigation Summary
At 12:30 p.m. on December 11, 2017, Employee #1 was adjusting the Omori wrapper to the new till size for the next production run. The employee left the guard up, and the safety interlock did not function. When the employee turned on the machine, it pulled the employee's finger into the cutting point. Employee #1's right index fingertip was amputated by the mechanical blade on the Omori wrapper.
