Investigation Summary
At 9:30 a.m. on December 18, 2017, an employee was operating a stand-up powered pallet jack. Just after making a right-hand turn down an aisle, he crashed into a storage rack. The employee's right foot became caught between the jack and rack, crushing two toes. The was hospitalized, requiring amputation of the two toes that were injured.
