Investigation Summary
At 8:30 a.m. on September 23, 2017, Employee #1 was using a jointer for jointing a 24 inch by 5 inch by 0.75 inch piece of wood. He was using a push block to advance the wood through the cutter head. The cutter head guard for the jointer was in place and operational. During the cut, the piece of wood dislodged, due to a knot in the wood, and the employee's right hand fingertips contacted the cutter head. Employee #1's index, middle and ring fingertips were amputated and he was hospitalized for treatment.
