Investigation Summary
At 12:00 p.m. on November 29, 2017, Employee #1 was operating an envelope machine at the feeder. The stock material became stuck at the underside of the machine conveyor belt, causing the belt to stop. The employee reached under the machine table with his right hand to remove the jam. The removal of the jam released the energy, and the movement of the belt drew his right hand into the nip point of conveyor and pulley, resulting in an amputation to the tip of the right hand middle finger.
