Investigation Summary
At 11:45 a.m. on November 10, 2017, Employee #1 was working under a jack to reinstall and realign the front axle of a Class 6 semi-truck. The jack was an American Forge and Foundry Air/Hydraulic Long-Chassis Jack, model 3135, which was holding the load at mid-axle. During work, the truck axle slipped off the cap of the jack and one end of the axle fell onto Employee #1's right leg. Employee #1 sustained a fractured leg and was hospitalized for more than 24 hours.
