Employee falls from stepladder and is killed

At 8:30 a.m. on December 19, 2017, an employee was working from a 6-foot Yellow Green Bell Fiberglass step ladder while installing insulation onto pipe joints in the Omni Hotel Mezzanine. The employee fell approximately 4 feet from the ladder onto the ground and was taken to the Irving Medical Center. The employee was killed.
