Investigation Summary
On December 18, 2017, Employee #1 was salvaging plastic gas tank from a wrecked car held up by forklift. Employee #1 pierced a plastic gas tank to drain gasoline into a bucket. Employee #1 spilled gasoline and it ignited. The employee sustained first degree burns to his left shin and left forearm and second degree burns to left side of his face. The employee was released from the hospital the same day.
