Investigation Summary
At 8:10 p.m. on December 13, 2017, an employee was cleaning a mixer by pulling out meat that was stuck in the machine. When the meat came loose, the paddle on the mixing machine moved. The employee's left hand was caught in the mixer. Three fingers on the left hand were amputated.
