Investigation Summary
At 5:30 p.m. on November 29, 2017, Employee #1 was helping guide the leaf vacuum truck that was dumping leaves in the company yard. The employee went behind the leaf vacuum truck while his coworker was backing up the vehicle. He was struck by the truck, and sustained a left torn ligament to his upper leg above his knee.
