Investigation Summary
At 7:00 p.m. on November 21, 2017, an employee was pushing a large FedEx container to get it as close as possible to a conveyor belt. The employee caught his left hand in an unguarded chain and sprocket gear and amputated four fingers. The employee was transported to Harbor-UCLA and transferred to LAC/USC where he underwent surgery and was hospitalized.
