Investigation Summary
At 9:15 p.m. on November 22, 2017, an employee was working at the conveyor in the frozen packing room. The conveyor did not work correctly, and the employee attempted to fix the conveyor by putting pie dough into the sprocket gear and chain. Her hand was caught into the gear resulting in a deep laceration from the palm to the back of her hand that required surgeries and one week hospitalization. The sprocket chain and gear were not guarded.
