Investigation Summary
On November 30, 2017, an employee was working for a firm that provided warehousing and storage. He was at his employer's retail distribution facility. He was performing a daily inspection of an AmbaFlex Spiralveyor SV spiral conveyor, with Serial Number 60249-05, on the facility's Loop Number 2. He was cleaning the conveyor system. While the conveyor was energized, the employee removed a guard. He removed the guard so that he could inspect the conveyor drive chain tension and look for broken gear teeth. The conveyor was not locked out during the inspection. To access the drive chain, he crawled beneath the conveyor after he had removed the guard. He was using his left hand to brace himself as he performed the inspection. He had a rag wrapped around his left hand. His left hand was positioned near the moving chain. The rag became entangled in the moving chain. The rag pulled his hand into the moving chain. The employee sustained an amputation of the tip of the middle finger on his left hand. It also said that the employee sustained the degloving of the tip of the index finger on his left hand. The narrative said that he was hospitalized for the degloving.
