Investigation Summary
At 2:00 a.m. on November 21, 2017, an employee had adjusted a chain with the electric-driven motor in the off position. The employee turned the motor on and then oiled and greased the chain and sprocket assembly. The employee grabbed a rag to remove some oil or grease from the center of the chain and sprocket and the rag was pulled into the machinery, catching his left hand in the in-running section of the smaller sprocket. The employee amputated three fingers and fractured two others on his left hand. One finger was reattached surgically and the employee was hospitalized.
