Investigation Summary
At 9:45 a.m. on November 27, 2017, Employee #1 was using an Eubanks 4600 wire cutting machine to cut wire ties. The machine became jammed, and Employee #1 reached into the machine to clear the jam. His right hand contacted the machine's cutting mechanism, resulting in the amputation of the tip of his middle finger.
