Investigation Summary
At 5:30 a.m. on November 22, 2017, an employee walked to the back room intending to get a hand wipe. The floor in the back room was wet as it was just mopped by another employee. The employee slipped on the wet floor just as she was reaching for the hand wipe. The employee's left leg went under her, bending back as she fell. The employee fractured her left femur.
