Investigation Summary
At 10:45 a.m. on November 14, 2017, Employee #1, employed by a construction company, was engaged in steel erection work at a multiemployer construction project, a two-story building. He was disassembling a RustGo aerial work platform when the platform ladder he was holding onto collapsed. Employee #1 fell with the ladder, a fall height of 12 feet. In addition to striking the ground, Employee #1 was struck by the falling ladder. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a fracture to the L5 vertebra and bruising to his chest and shoulders.
