At 4:30 p.m. on November 3, 2017, an employee was loading materials into a dye when a piston dropped by itself and compressed the employee's fingers between the still cup and the cylinder shaft. The employee fractured her hand as well as amputating two fingertips from her index and middle fingers. The employee was hospitalized.
