Investigation Summary
At 10:45 a.m. on September 21, 2017, Employee #1 was cutting down a 30 feet tree on a private residence. The employee was approximate 20 feet up, tied off in the tree, with the employer on the ground holding the guideline. As the employee made his second cut, he signaled for the employee to pull the rope. When the employer pulled the rope, the tree trunk broke below the suspended employee, causing him to fall with the tree. Employee #1 sustained fractured ribs, a broken clavicle, and a punctured lung, and was hospitalized for treatment.
