Investigation Summary
At 1:30 p.m. on October 28, 2017, an employee had partnered with one of the instructors during a Workplace Violence Defense class and was demonstrating a defense move called a safety gap. The employee was to be the aggressor and the moves were practiced several times with the class instructor. The instructor pushed the employee and the employee lost his balance and fell backwards, fracturing his left radius. The employee was hospitalized.
