At 6:30 p.m. on October 23, 2017, an employee was using a machine to place beads into sheet metal and moved from a machine that he was trained on to another machine, for which he had no training. The employee caught his left hand fingers in the roll's forming wheels and amputated them. The employee was hospitalized.
