Employee killed when caught in wood chipper

On March 15, 2001, Employee #1 was feeding an approximately 8-ft-to-10-ft-long brush into a wood chipper. The second employee was trimming brush in a tree and the third worker was removing brush from the ground. The second employee spotted Employee #1's body being fed into the wood chipper and shouted to the third worker who was removing brush on the ground approximately 30 ft from the chipper. He ran to the wood chipper and hit the safety bar, which reversed the feed rollers. Employee #1 sustained amputation of his head and died at the scene.
