Employee's fingers amputated in rotary blade

Employee #1 was using a water hose to clean excess materials from a transfer case without realizing that a rotary blade was running directly under it. He had placed his hand in the transfer case to check for remaining deposits when it became caught by the rotary blade. His left index and middle fingers were amputated, and his ring finger was partially amputated.
