Employee injured in fall when ladder slips out

Employee #1 and a coworker were carrying a spring system up a pair of ladders in an area where buses were serviced. The ladders were set side by side on the concrete floor. When the workers were at a height of approximately 14 to 15 ft, the footing of one of the ladders began to slip out. Employee #1 fell to the floor, injuring his leg and arm.
