Investigation Summary
Employee #1 and his supervisor were on a 40 ft long pump jack scaffold, removing and replacing shingles on the mansard roof of an apartment building. The scaffold was supported by aluminum poles at the ends and by a wooden pole of spliced 2 by 4 lumber in the middle. They were working at a height of 19 ft 6 in. when the wooden pole gave way and they fell. Employee #1 fractured his pelvis, and his supervisor fractured his jaw and right ankle. Both workers were air evacuated to the Shock/Trauma Unit in Baltimore, where they were admitted for treatment. The scaffold had no guardrails, and neither Employee #1 nor his supervisor were using a lifeline.
