Investigation Summary
Employee #1 and a coworker untied an aluminum extension ladder, model #AE2240, manufactured by the Louisville Ladder Corporation, that had been set up through a 3 ft by 2 ft 4 in. floor opening to access a 26 ft deep wet well. The 40 ft ladder extended when they attempted to pull it out of the floor opening to remove it from the wet well. Employee #1 got on the ladder and then climbed down several feet to tie the rope of the ladder around the rungs to prevent it from extending any farther. As he did so, he fell approximately 22 ft 6 in. onto the concrete floor of the wet well. He sustained fractured ribs and internal injuries that later led to his death. No defects or damage were found in the ladder. The employer had a written safety and health program, provided training to employees, and had written certification of Employee #1's being trained in ladder safety.
