Investigation Summary
Employees #1 and #2 were rehabbing a balcony that was 17 ft 6 in. above the ground. They had broken up the concrete with a jackhammer and were preparing to get onto the personnel lift when the balcony collapsed. Employees #1 and #2 fell, sustaining injuries that required hospitalization. Neither employee was tied-off, nor had they been trained to work on elevated surfaces.
