Investigation Summary
Employee #1, working next to a water well drilling rig, bent down next to a truck to connect a water line. A 20 ft long by 3 in. diameter steel drill rod was being hoisted between 7 and 8 ft off the truck when it fell and struck Employee #1. He sustained head lacerations, for which hewas hospitalized. Employee #1 was not wearing a hard hat., and the drill rod was not rigged properly.
