Investigation Summary
Employee #1 was working next to an unguarded wall opening that was 109 in. wide by 69 in.. high and 10 ft 4 in. above the ground. The bottom of the opening was 13 3/4 in. above the floor. Employee #1 was using a crowbar to pry a window header. The header cracked and the momentum caused Employee #1 to fall through the unguarded opening. The employee was not using fall protection and he had not been trained on how or when to use it. He sustained a fractured left orbital and two fractured wrists when he struck frozen ground.
