Employee killed in fall when ladder slips out

Employee #1 was working in the furnace area and wanted to talk to some coworkers who were laying bricks inside the furnace. He climbed a portable ladder that was held by a coworker and exited onto the top of the furnace. The coworker then left to prepare for other work. Employee #1 talked to coworkers at the furnace for several minutes and then started down the ladder, despite the fact that nobody was there to hold it. The ladder started to slide out from under him, so Employee #1 jumped clear. He fell between 6 and 8 ft onto a concrete floor, and suffered a fractured skull. Employee #1 was killed.
