Investigation Summary
At 11:115 a.m. on October 24, 2017, an employee was working in a pole barn at a minimum security prison camp and was ripping a 2 by 4 by 8 board on a Dayton 10 Contractor Table Saw, model # 4KXD1, lot no. 051200021, for the construction of a shooting range target frame. The employee was working alone and, as the employee cut the board, his left hand came into contact with the saw blade and he amputated his left hand's middle and index fingers. Hospitalization was not required.
