Employee fractures heels from fall

An employee was wearing a defective Y-Chain rebar assembly lanyard while setting concrete gang forms. After securing the assembly device to the channel stiff back, the lanyard device failed, causing the employee to fall approximately 10 feet to the ground. The employee fractured his heels.
