Employee #1 was working on an elevating work platform installing plumbing fixtures. At the time of the accident, Employee #1 was working on a drinking fountain, separating a drain line from a storm/sanitary combination sewer. Equipment being used as a door carrier started moving and crushed Employee #1 killing him.
